VIVE Programs

VIVE Home and Heath Services is accredited by the Texas Health and Human Services Commission to provide personal care in South Texas to eligible individuals of all ages. We accept all types of Medicaid insurance carriers and private pay.

Our program members receive exceptional home supportive care and value added services tailored to their needs and preferences. Our VIVE Kids Club is specifically designed for members under 21 years of age. Our Adult Home Care Solutions program is specifically designed for our adult members 21 - 100+ years of age.

Our valued employee team also have a dedicated support system to help them outperform in their duties, enhance their well-being, and meet their long-term goals.

• Person-Centered and Age-Appropriate Care Plans

  With our shared-decision making platform, person-centered and age-appropriate, services and supports are put in place to help your child thrive.

• Professional Concierge Services

  We enhance your overall service experience, and you have prompt access and response from a dedicated care specialist in addition to your selected home care provider.

• Daily Living Assistance

  Customized support with hygienic needs, meal preparation, feeding, body positioning, transferring, or accompaniment to the doctor.

• Light Housework and Organization

  This may include an array of tasks such as making the bed, cleaning member's living space, laundry, taking out the trash, making a list and shopping for groceries and household items needed, storing purchased items, or picking up medication.

• Specialty Care

  These individualized and specialized services may range from transitional care, respite care, surgery assistance, chronic disease care, and end-of-life support care.

• Value Added Services

  A range of essential, protective, and supportive services available. The goal is to ensure everyone has access to the tools and resources necessary to lead a safe and fulfilling life. Examples include and are not limited to:

  • School services

  • Family support services

  • Coordination of care

  • Service-related correspondence review and assistance

  • Help with filling out forms for benefits, services, or devices

  • Welcome gift, care packages, and

  • much more.
